DIGNITY HOUSING, A SOCIAL SERVICE AGENCY SEEKS MOTIVATED AND ENERGETIC INDIVIDUAL
Position Summary:
The Case Manager (CM) is responsible for providing case management support including home-based visitations and office consultations ensuring that basic needs of residents are met. Case Managers are required to conduct basic needs assessments and work with Dignity Housing residents to develop a service plan delineating goals and action steps toward self-sufficiency. The CM is also responsible for monitoring progress, updating the service plan, providing referrals and resources related to basic family needs. The CM is required to work in collaboration with other Social Service Staff and Director of Program Operations. The CM will provide case management and supportive services to a caseload of no more than 20 residents. This position is part-time, 30 hours per week. Salary is $15-18 per hour, depending on exp. Some evening and Saturday work required.
Qualifications:
Bachelor’s Degree in related fields required. BA degree in Social Work preferred. At least 2yrs Experience in D&A treatment/prevention and recovery a must. Familiarity with standard case management procedures; basic knowledge of government benefits and community resources; experience in case management/socials service delivery; knowledge of addiction and mental health pathologies; Proficiency in most general computer operation systems & applications required, including: Word, Excel, Outlook, Publisher & Power Point.
Additional Requirements:
Valid Driver’s License, vehicle with insurance and current criminal background and child abuse clearances. Maximum of two page writing sample and three professional references also required with resume.
