Bridges Project Medical Case Manager
The primary duties of the Medical Case Manager includes: community outreach, intake, case management, advocacy, service/linkage coordination and treatment adherence for limited English proficient people living with HIV/AIDS.
Major Responsibilities:
• Provide direct services including case management and treatment adherence services for clients and their families.
• Conduct client intake, periodic assessments, and care planning for clients and document all delivered services.
• Recruit clients through community outreach and program promotion with agency’s education & outreach activities.
• Develop collaborative relationships and coordinate linkages with AIDS, health, and other social service providers.
• Maintain records and prepare program reports.
• Handle referrals for testing program.
• Assist in program and agency compliance with funding guidelines, contracts, policies, regulations, etc.
• Other duties as assigned
