Assistant Health and Disabilities Coordinator, Maternity Care Coalition
An Assistant Health and Disabilities Coordinator is needed for the Early Head Start program to assist in the planning, supervision, coordination, implementation and monitoring of required services to participating pregnant women and their families, as well as staff in the Health and Disabilities Service Areas in Pottstown, PA. The Assistant Coordinator works in linking families to an ongoing system of health care, follows up with treatment services for medical and dental health issues; provides health resources for the families ensuring that families are linked to a medical home; maintains the system for storing health records and other vital health service data; assists in the promotion of health and safety practices, including performing quarterly health and safety checks.
We seek a motivated individual, knowledgeable about community health and child health and development issues, with good computer skills. A valid driver’s license, clean driving record and a minimum of an Associate’s Degree in health or related area with prior experience in health service provision is required. Bachelor’s Degree preferred. The position is in Pottstown, PA.
