Aging at Home – A Community Network

United Way of Greater Philadelphia and Southern New Jersey (UWGPSNJ) has launched a new model of community-based aging called Aging at Home – A Community Network to help seniors age in good health in their homes and communities at Congreso de Latinos Unidos in eastern North Philadelphia.
Congreso de Latinos Unidos is one of three coordinating agencies selected to implement this new aging model in the Philadelphia Region – through VIDA at Congreso. Senior Community Services in Lansdowne and VNA Community Services, Inc. in Abington are two other partners in this work.
These local organizations will develop relationships with strategic community providers, stakeholders from the community, policy-makers, and health and social service agencies to coordinate access to local social and health services such as health screenings, chore assistance, transportation and volunteer placement opportunities.
Through Aging at Home – A Community Network, more older adults living in the region will now have coordinated access to the community-based services they need to improve their health and quality of life.
