Maternity Care Coalition (MCC), a dynamic nonprofit, works to improve maternal and child health and well being through the collaborative efforts of individuals, families, providers and communities.
MCC seeks a self motivated individual to fill a position in our MOMobile program. The Advocate will conduct home visits with pregnant and newly parenting women to provide education on health related topics, parenting, and prenatal care. Maintains contact with clients to ensure connection to needed healthcare and other community resources; provides additional support to clients needing more intensive follow-up and makes referrals to appropriate agencies. The Advocate will learn about and make contact with community resources needed by clients, participate in community and parent meetings, outreach events, and in trainings. Will use organizational database and keep accurate records of all contacts and services provided. Interest in maternal and child health and ability to work in a team environment desired. High school diploma and valid driver’s license with clean driving record required.
