Adolescent D&A Case Manager
Congreso de Latinos Unidos, Inc., a dynamic nonprofit, is actively seeking an individual to fill the position of Adolescent D&A Case Manager. The Adolescent D&A Case Manager will provide screening, assessment, and case coordination for youth struggling with addictions. Screening includes evaluating the adolescent’s need for a referral to emergent car including detoxification, prenatal, perinatal, and psychiatric services. Assessment includes Level of Care assessment and placement determination. The case manager will link clients to treatment and provide direct service recovery support/social services. Through trauma-informed case coordination, the CM ensures that the adolescent’s treatment and non-treatment needs are addressed. The case manager (CM) will conduct outreach activities to build and maintain a case load, engage actively with clients and provide quality services, goals, contract compliance and reporting.
The case manager will have internal contact with other programs in Congreso, administrative staff and external contact with participants, community resources and schools. The case manager will report directly to the Manager of Health Education and training supervisor and during her/his absence, to the Director of Woman’s Wellness.
This position has access to sensitive Congreso information and is expected to handle such information with integrity, privacy, and professionalism. This position also has regular contact with members of the community and is expected to represent Congreso in a professional manner.

Essential Responsibilities
1. Responsible for recruitment, retention, and management of a caseload of 35 cases.
2. Provide trauma-informed and age-appropriate support to adolescents struggling with addiction.
3. Provide all client with recovery supportive services such as: linking clients to treatment, assisting in navigating the health and social systems and ensuring that clients are connected to education and employment activities, attending recovery meetings with clients as necessary, working hand in hand with the client’s therapist to secure that he/she is on track
with their goals and needs.
4. Working with the client’s family to educate them on ways to support the recovery process and linking families to supportive services.
5. Provide case management services within the Primary Client Model (PCM) philosophy and model to all target population participants.
6. Assist with the planning and coordination of outreach and marketing to schools, after programs, clinics, outpatient D&A facilities, and other agencies to recruit participants for services.
7. Participate in Case management supervision, trainings and meetings mandated by the agency, funders and assigned by supervisor.
8. Maintain clear, concise and complete records in accordance with Congreso and DDAP (Department of Drug and Alcohol Program) requirements.
9. Complete monthly and quarterly reports internally and externally as assigned.
10. Complete and submit to DDAP all mandated documents related to incident reports that may occur involving any participants.
11. Provide flexible hours for client appointments in accordance to program requirements.
12. Support other Health Education programming, including drug and alcohol prevention education, as part of a holistic team.
13. Other duties as assigned.
Minimum Experience:
1-2+ years of related experience as case manager in the area of substance abuse. Experience working with at-risk adolescents providing screening and assessment for addiction.
Minimum Education:
BA degree from an accredited college or university in the field of study of Social Work, Counseling, Psychology or a related field preferred.
