2019 Philadelphia Resident Survey

The City of Philadelphia needs to hear from you! We would like to have as many individuals as possible complete the 2019 Philadelphia Resident Survey. It would be extremely helpful if any of your service members could support this effort! Here is a form to fill out which will let us know if and how you can participate: https://forms.gle/viSvGQSHJqJJ4KSM8 . Everyone is encouraged to take the survey, including you! Here is the link to the survey online: www.phlsurvey.com, we also have drafted email language as well as physical survey packets and FAQs which we will distribute to your programs.
