<p><strong>About the organization</strong>: The Penn Center for Community Health Workers (PCCHW) is a national center of excellence for community health worker research, application, and dissemination. We developed IMPaCT, an evidence-based community health worker model, which has been proven in clinical trials and replicated nationally and has supported almost 25,000 historically underserved patients in Philadelphia. The results demonstrate improvement in chronic disease control, improved mental health, improved communication between patients and providers, and 34% fewer hospital days compared to matched controls.</p>
<p>CHWs are trusted members of local communities who promote health and well-being by supporting patients&#8217; physical, mental, and emotional goals. They are hired and trained based on key characteristics, including innate qualities like good listening, empathy, and trustworthiness with the patients they support.</p>
<p><strong>Key Duties and Responsibilities:</strong></p>
<p><strong>Organization leadership</strong></p>
<ul>
<li>Oversee programmatic outcomes and continuous improvement for patient care across hospital, outpatient, and telephonic CHW work and ensure top CHW performance</li>
<li>Manage PCCHW’s team training and CHW professional development</li>
<li>Integrate CHWs into hospitals and primary care clinics and build strong relationships with clinical partners.</li>
<li>Represent the Center in major health system initiatives related to population health and social determinants of health (SDOH).</li>
</ul>
<p><strong>CHW leadership</strong></p>
<ul>
<li>Supervise up to 2 Program Managers who lead a team of CHWs</li>
<li>Supervise and coach 3 CHWs to provide excellent care</li>
<li>Analyze data on a regular basis and identify opportunities for improvement</li>
<li>Develop and implement performance improvement strategies and plans</li>
</ul>
<p><strong>Minimum Requirements:</strong></p>
<p>Required Education and Experience:</p>
<ul>
<li>A Bachelor&#8217;s Degree is required; a Master&#8217;s is</li>
<li>8+ years of experience in a management role with a track record of hiring and retaining superstars, and achieving program goals, including 5+ years of leadership experience in organizations focused on improving the lives of individuals who have experienced harm due to race and socioeconomic position</li>
<li>Self-starter who can make strong connections within and outside of the Center while working in a hybrid in-person/remote office arrangement</li>
<li>Comfort and enjoyment working with data to understand trends, troubleshoot, and innovate</li>
</ul>

<p><strong>About the organization</strong>: Penn Center for Community Health Workers is a center of excellence for CHW research, patient care and dissemination. Our IMPaCT model, which has been featured on National Public Radio and the New York Times, has served more than 8,000 people in Philadelphia. The program is proven to improve primary care access, chronic disease control, and mental health while reducing hospital days by 65%. More than 1,000 organizations have accessed our tools and we provide technical assistance to help organizations create, launch and sustain effective CHW programs.</p>
<p><strong>Key Duties and Responsibilities:</strong></p>
<p>The IMPaCT Trainer is a member of our Training and Technical Assistance team. (S)he will:</p>
<ul>
<li>Use the evidence-based IMPaCT model to deliver engaging, effective training to CHWs working in healthcare and community organizations across the country</li>
<li>Provide individual and group feedback to help CHW trainees learn</li>
<li>Refine course delivery, including developing online learning content, to help continually scale our work</li>
<li>Be the face of IMPaCT to our partner organizations, building the foundation for long-term, productive partnerships</li>
<li>Provide technical assistance and tailored troubleshooting to partner organizations, supporting fidelity to the best practices</li>

<p>General Job Statement:</p>
<p>Every day, we care for low-income patients in our hospitals and primary care clinics. We try our best to keep them feeling well so that can live their lives to the fullest. But life can be tough and lots of things can keep people from staying healthy and from getting better when they are sick. Patients have worries such as: “How will I get to the pharmacy to get my prescriptions filled?”, “Who will watch my children while I go to the doctor?” and “Will I have food, transportation and housing?”</p>
<p>We believe that community-health workers (CHWs) have the skills and experience to understand what low-income patients are going through and help them get through difficult times. CHWs are people who come from the communities they serve. In the IMPaCT program, CHWs act as caring neighbors to help patients address the social and medical problems that lead to poor health. The goal is to help patients out with the tasks of getting medical care, working on health goals (arranging care, filling medication prescriptions, planning healthy meals or finding time to exercise), and to help them deal with the social issues that keep them from staying healthy.</p>
<p>Minimum Qualifications:</p>

<li>Great with people! Able to build strong relationships</li>
<li>Comfortable with home visits and outreach</li>
<li>Ability to use good judgment in meeting job responsibilities</li>

<p>Preferred Qualifications:</p>
<ul>
<li>Long-time resident of West or Southwest Philadelphia and good knowledge of the resources of these communities.</li>
<li>Prior experience as an outreach worker.</li>
</ul>
<p>Key Duties &amp; Responsibilities:</p>
<ul>
<li>Attend a month-long paid training program</li>
<li>Meet patients in the primary care clinic or hospital and conduct a needs assessment, including helping patients to set health goals</li>
<li>Make weekly follow-up calls and home visits to patients</li>
<li>Motivating patients to meet their health goals</li>
<li>Helping patients with social issues like homelessness, substance abuse and hunger</li>
<li>Assist patients with organizing their records, making follow-up appointments, and filling their prescriptions</li>
<li>Help patients fill out applications for Medical Assistance and SNAP (Supplemental Nutrition Assistance Program)</li>
<li>Work with other CHWs and staff to create a directory of community resources (e.g. foodbanks, housing assistance programs, childcare resources, etc.) in West and Southwest Philadelphia.</li>
<li>Document each patient encounter in detail</li>
<li>Prepare reports and documents as needed or requested.</li>
<li>Attend a weekly group meeting with other CHWs, and program supervisors</li>
<li>Other duties as reasonably assigned.</li>
