Dismantling Disparities in Communities: Resources and Tools to Advance Systems Change

Regardless of focus—whether on cradle-to-career education, health, transportation, economic development, or other topics—successful cross-sector partnerships require strong guiding principles, system infrastructure, and organizational strategies and practices. But how can community partnerships best assess their strengths in these areas, and how can they identify where to go deeper to effect place-based systems change?

With this tool, Mathematica and Equal Measure provide a framework to help community partnerships assess their readiness for systems change, along with a series of practical worksheets.

The tool is for any cross-sector systems change practitioner, funder, or technical assistance provider interested in examining a partnership’s current strengths, areas for improvement, assets, and barriers. It can support decisions around upcoming investments, technical assistance needs, and site selection. It also codifies a cohesive framework for measuring place-based systems change efforts.
