Public Health Nurse Home Visitor, Nurse-Family Partnership

The Public Health Nurse Home Visitor is responsible for providing comprehensive in home and community based nursing services to women and their families eligible for the NFP program in Philadelphia. The Nurse Home Visitor maintains the highest standards in clinical practice and adheres to the NFP model. The Nurse Home Visitor upholds the policies, procedures, guidelines and standards of NFP and the lead agency. The Nurse Home Visitor reports to the NFP Supervisor and the Nurse Administrator regarding clinical practice and client/program outcomes. The Nurse Home Visitor with the Supervisor, Administrator and senior staff works collaboratively to build and improve the strengths-based client-centered culture. The Nurse Home Visitor with administrative and supervisory support is a spokesperson and advocate for public health, nursing and preventive health initiatives that supports family and community well-being.

Responsibilities:
Acquires and applies in clinical practice the educational foundation in Nurse-Family Partnership program goals, theories, practice principles, rules of engagement and model elements by completing all required NFP and Early Childhood education. Supports the policies, procedures, guidelines and standards of NFP and the lead agency. Adheres to nursing process and the NFP model of home visitation and community engagement. Participates in weekly clinical conferences and staff meetings. Provides information to support staff for accurate data input and data summaries for required data bases. Contributes to professional development seminars and weekly clinical conferences. Consults and collaborates with other professionals involved in providing services to women and their families. Contributes to a positive work environment that enables all staff to function at their highest potential. Collaborates with staff regarding community referral sources and disposition of referrals. Engages in student learning work at undergraduate and graduate levels. Assumes other responsibilities as needed.

• Has basic knowledge of community resources and interest in learning about relevant health, education, housing, social services and early childhood programs
• Possesses strong interpersonal skills and the desire to work in collaborative public health model

Experience:

• Three or more years of experience working in community/public health with urban, underserved populations in some capacity highly preferred
• One or more years of nursing experience working in maternal-child health, early childhood, home visitation mode, and health education preferred. New graduates are welcome to apply.

Education Requirement:

Baccalaureate Degree in Nursing from accredited university required. Master’s degree in Nursing, Public Health, Community Systems, Maternal-Child Health or related field preferred.
