Legacy Projects Seed Grants

Legacy Projects (LP) are non-renewable seed grants provided by the Southeastern African American Diabetes, Hypertension and Stroke REACH US Center of Excellence for Eliminating Disparities (SEA-CEED).  The purpose of this seed grant program is to support and empower communities working on local solutions to eliminate health disparities, specifically related to Diabetes Prevention and Control and associated hypertension, stroke and amputation risk factors in African Americans/Blacks.  The long-term goals for the LPs are to help build the capacity of the affected community and create self-sustaining, community-based partnerships that will continue to improve local conditions in the future.  Proposed projects should include a Community Action Plan (CAP) that includes strategies for identifying local issues, educating and empower the community about the issues, building consensus, and setting community priorities.  In addition, the CAP should also demonstrate how the applicant will collaborate with other stakeholders to realize their project goals and objectives.
