Position Summary:
The Case Manager (CM) is responsible for providing case management support including home-based visitations and office consultations ensuring that basic needs of residents are met. Case Managers are required to conduct basic needs assessments and work with Dignity residents to develop service plans (Life Skills Action Plans – LSAP) delineating goals and action steps toward self-sufficiency; monitor progress and update LSAPs every ninety (90) days; provide referrals and resources related to basic family needs (income, mainstream benefits, childcare and youth services, parenting, health and wellness, transportation, education, financial planning, housekeeping, etc.). The CM is required to work in collaboration with other Social Service Staff to ensure comprehensive services and supports are being provided to residents. The CM will provide services and supports to a caseload of no more than 20 homeless individuals and families. This position is part-time 30 hours per week. The position pays $14.00/hour.
Qualifications:
Familiarity with standard case management document collection procedures; Basic Knowledge of Government Benefits and Community Resources; minimum of 3-5 years experience in direct case management delivery; knowledge of addiction and mental health pathologies; Proficiency in most general computer operation systems, including: Word, Excel, Outlook, Publisher, & Power Point required.
Additional Requirements:
Valid Driver’s License, insurance, and vehicle in good repair required for the position. Current criminal background and child abuse clearances required. Writing sample required with application for employment. Minimum of four professional references required, unless otherwise indicated.
