POSITION SUMMARY:
ASIAC (Formerly AIDS Services in Asian Communities), a program partner of the Urban Affairs Coalition (UAC) is currently looking for an individual to fill the part-time position of Bridges Project Medical Case Manager.
The position of medical case manager serves to primarily implement the Bridges Project which includes: community outreach, intake, medical case management, advocacy, and service/linkage coordination for primarily Limited English Proficient people living with HIV/AIDS. Candidates with HIV CTR Certification will assist with occasional agency testing events. This position reports directly to the Executive Director.

MAJOR RESPONSIBILITIES: Provide direct services including medical case management and treatment adherence services for clients and their families; Conduct client intake, periodic assessments, and care planning for clients and document all delivered services; Connect clients into medical care; Recruit clients through community outreach and program promotion with agency’s education & outreach activities; Develop collaborative relationships and coordinate linkages with AIDS, health, and other social service providers; Maintain records and prepare program reports; Coordinates referrals for clients; Assist in program and agency compliance with funding guidelines, contracts, policies, regulations, etc.; Other duties as assigned.
MAJOR QUALIFICATIONS AND SKILLS: Bilingual (English and Indonesian) language skills strongly preferred. Reading, writing, and interpretation; One year of case management experience; One of the following credentials: Bachelor in Social Work, Social Science, Science in Nursing or equivalent or Registered Nurse licensed in PA with one year experience and a combination of 12 semester hours in psychology or other social welfare courses; Documented case management training; Enrolled in the Medical Assistance Program;

Knowledge of HIV transmission and psycho-social issues related to risk behavior including sexual and substance abuse behavior; Must be sensitive, friendly, and have “people skills” to work with clients including those who are sexual minority, sex workers substance abusers, low-income individuals, and immigrants/refugees; Counseling skills & support group facilitation skills preferred; Excellent communication, computer and interpersonal skills; Must be highly organized, able to work under pressure, dynamic, creative, and self-motivated.
