TITLE: HIV Medical Case Manager (Full–time – 35 hours per week)
(Opening Date: November 7, 2011)
REPORTS TO: Program Manager
POSITION SUMMARY:
The Gay and Lesbian Latino AIDS Education Initiative (GALAEI), a program partner of the Urban Affairs Coalition (UAC) is currently looking for an individual to fill the position of HIV Medical Case Manager.
The HIV Medical Case Manager provides individualized case management services to HIV-positive clients. Specific duties include: intake, assessment of client needs, care planning, advocacy, service linkage, and outreach and education. Delivers individualized case management from a harm reduction and culturally appropriate perspective and participates in ongoing clinical supervision as part of a team of two medical case managers.
PRIMARY RESPONSIBILITES:
Conduct client intake, periodic assessments, and care planning for clients;
Document all delivered services – obtain mental health, clinical care, housing, public benefit, or other services needed to improve the client’s physical and emotional health – keep individual client charts;
Advocate on behalf of the client to assure appropriateness of services and client compliance with requirements;
Develop collaborative relationships and ensure for the coordination of AIDS services, health, and other services for the client;
Provide non-judgmental, harm reduction focused education and additional resources around sexually transmitted diseases;
Provide safer sex materials and risk reduction, skills building education to client;
Participate in general community outreach efforts of the agency;
Participate in program activities, including program promotion, outreach, client recruitment, clinical supervision, etc.;
Encourage the involvement of clients in other community activities which encourage understanding of secondary HIV prevention, community stability, and affirmative lifestyle choices;
Complete administrative duties including filing, shredding, collating, stocking, recording results, supply inventory, daily computer data entry and other operational needs related to the case management effort;
Arrange interpretation and translation services as necessary;
Maintain client records and prepare monthly program report and data submissions to CareWare;
Adhere to agency compliance with contracts, policies, regulations, confidentiality policies, HIPPA, etc.;
Other duties as assigned.
QUALIFICATIONS AND SKILLS:
A minimum of bachelor’s degree in social work or social sciences. Candidates not meeting this requirement will not be considered;
Bilingual (English/Spanish) experience strongly desired. MUST be able to read, speak, and write in English fluently;
Must possess knowledge of HIV transmission and issues related to risk behavior including sexual and substance use behaviors;
Must be sensitive, friendly, and have the “people skills” to work with clients including those who are lesbian, gay, bisexual, transgender, sex workers, substance users, low-income individuals, and immigrants/refugees;
Counseling skills and group facilitation skills preferred;
Excellent communication, computer and interpersonal skills required;
Must be highly organized, able to work under pressure, dynamic, creative, and self-motivated.
