General Description:
Case Manager/Supervisor will have a minimum of a Bachelor’s degree and will direct and oversee the day to day operations of the DHS Diversionary Case Management Program and its staff. The Case Manager/Supervisor shall be responsible for contract administration, development, and monitoring of the cases, activities, and schedule for the overall program. Case Managers are required to locate and pool resources, sequence and coordinate services/resources responsive to assessed needs/strengths, and monitor the services delivery and needs of their family center member caseload. Conduct assessment/family service plans. Perform home visits keep up to date, the FACES Data Based with all the details and information about the support services provided to the family. Attend mandatory trainings and meetings from DHS, attend mandatory trainings, meetings, and events held at Norris Square Civic Association.
Qualifications:
1) Bachelor’s Degree in Social Work or related field and/or equivalent experience.
2) Minimum of 3 years supervisory experience.
3) Excellent verbal and written communication skills in Spanish and English.
4) Demonstrates experience in proposal writing.
5) Strong leadership and supervisory skills.
6) Commitment to community empowerment.
7) Ability to lead and to motivate others to do so.
8) Ability to teach and model.
9) Computer Literate.
10) Ability and willingness to work evenings and weekends.
