Description:
Our Senior Services team is looking for an experienced social worker to provide in-home care management and supportive counseling to older adults in Greater Philadelphia so that they can continue to live safely in their own homes. Responsibilities include:
• In-home psychosocial assessment with the older adult client, involving family caregiver as appropriate
• Working collaboratively with the client and caregiver to develop a realistic long term care plan which links the client to benefits and services
• Assisting the client and caregiver with implementation of the care plan
• Monitoring the client’s situation via phone calls and home visits
• Documenting client activity on JFCS client data base system
The qualified candidate will have:
• MSW or equivalent degree and experience, LCSW license preferred
• At least three to five years experience working with older adults, preferably in the community
• Ability to demonstrate strong working knowledge of community resources and benefits relevant to older adults and their family caregivers
• Ability to use Microsoft Office 2007 including Word, Excel and Outlook
• Ability to communicate effectively with a variety of audiences, adjusting message to fit listener to ensure understanding
• Availability of car, current driver’s licenses and auto insurance, and willing to drive throughout Greater Philadelphia
