Truancy Case Manager, Intensive Delinquency Prevention Program

REPORTS TO: Education Department Co-Director
JOB SCHEDULE: Part-Time (flexible scheduling) – Maximum 20 hours per week from November 1, 2010 through January 31, 2011
PRIMARY FUNCTION(S):
SEAMAAC’s Intensive Delinquency Prevention program works in partnership with Philadelphia’s Department of Human Services (DHS) to improve school attendance and academic achievement through case management services for at-risk youth as well as their families. The Truancy Case Manager will work with the Intensive Delinquency Prevention team to offer support and advocacy services primarily focused on students involved with truancy court proceedings.
The Case Manager is responsible for ensuring ongoing support for youth and their families through assessments to identify strengths and weaknesses, referrals and linkages to services, evaluation of service effectiveness and any additional support that will benefit the students and their families. The program’s goal is to increase opportunities for youth, build positive relationships in their family, school and communities, and help them achieve their potential.
ORGANIZATIONAL CONTEXT:
SEAMAAC’s mission is to support immigrants, refugees and their families as they seek access to opportunities which would advance the condition of their new lives in the United States. In the Education Department, SEAMAAC offers a continuum of services to children and their families that honors cultural heritage and promote leadership. Programming includes After-school/Summer Camp and Leadership Development for youth in Elementary and High Schools, family/community dinners and door-to-door outreach campaigns, intensive case management as well as adult ESL programs. Through collaboration with the Health and Social Services Department, SEAMAAC also offers intergeneration programming for elders and youth.
QUALIFICATIONS
•Bachelor’s degree required in Social Work, Education, or related field.
•Minimum one years’ case management or clinical experience.
•Experience and connection to the immigrant/refugee community.
•Knowledgeable about the issues facing Southeast Asian communities in particular and immigrants in general in urban settings.
•Experience working with Limited English Proficiency population preferred.
•Fluency in Spanish is required.
•A desire to serve the community.
•Excellent communication skills, both oral and written as well as excellent computer skills.
•Demonstrate initiative and problem solving skills.
•Demonstrated ability to work and communicate with diverse staff.
•Demonstrated ability to work in a team environment.
•Demonstrated ability to handle a heavy workload, exercise discretion, maintain confidentiality, handle multiple priorities and use sound judgment.
•Demonstrated ability to problem-solve.
•An ability to travel around local neighborhoods extensively.
•Criminal, Child, FBI and health clearances required.
DUTIES, RESPONSIBILITIES AND EXPECTATIONS:
•Support the children and families in a manner that fosters independence, growth and development and will facilitate the youth and families achieving their stated goals.
•Prepare for and attend court appearances to represent clients at South Philadelphia Truancy Court and Family Court.
•Accurately assess the strengths, supports, and requirements of the identified children, families and communities in which they live.
•Maintain consistent communication through school visits and home visits with families and provide on-going support to students as needed.
•Provide direct support as well as referrals to appropriate agencies for additional services to achieve goals. Ensure that referrals are completed promptly and confirm that a linkage between the family and provider is secured.
•Engage individuals and organizations for the purpose of working in partnership and collaboration.
•Document all activities directly or indirectly related to the client in case files and database.
•Exercise discretion and maintain staff and client confidentiality.
•Attend regular supervision and department meetings.
•As part of the Education Department that promotes lifelong learning, staff is expected to set the example and to continue learning through SEAMAAC provided trainings.
•Understanding of, and commitment to, SEAMAAC’s principles, concerns, and considerations in regard to issues of race, class, nationality, religion, age, gender and sexual orientation, and disabilities.
•Actively participate in organizational-wide and community work efforts and events.
•Flexible hours and may require some travel and nighttime or weekend assignments.
•Other assignments, including translation/interpretation, as required.
•And such other duties as may be assigned from time to time.
