Job Description:
Incumbent will provide direct services to consumers; ensures quality program services for consumers within areas of responsibility. Perform other duties as assigned.

Required Education and Experience:
Successful completion of 12 credit hours of human services course work from an accredited educational facility and at least four years of professional experience or two years of professional experience and two years as a Home and Community based waiver program participant. Additional educational experience may substitute for the given requirements. Incumbent must possess excellent communication skills (written and verbal); strong organizational skills and computer literacy. Incumbent will be required to successfully complete resource management training within first 90 days of hire.
