SENIOR CASE MANAGER-ST. COLUMBA

The primary responsibilities of the Senior Case Manager are to supervise Support Staff, provide assistance to and back-up for the Program Manager, oversee a designated caseload and oversee activities related to the care and welfare of the Residents. Senior Case Manager will assist the Program Manager with the oversight, training, implementation and use of the new best practices (Motivational Interviewing, Critical Time Intervention, Wellness Self Management and Cognitive Remediation). The Senior Case Manager is responsible for the general oversight of all medication monitoring activities including medication procurement, documentation and tracking/reporting medication errors.

EDUCATIONAL REQUIREMENTS:

· BA, BSW, or related degree. Masters preferred.

EXPERIENCE REQUIREMENTS:

· A minimum of 3 years of prior case management experience working with clients with a history of substance abuse, mental illness & homelessness

· At least one year of prior supervisory experience

· Strong verbal & written communication skills

· Knowledge of case management systems

· Good computer skills (Microsoft Office Exel, Word, etc.)

· Prior knowledge & sensitivity of working with people with mental illness, dually diagnosed and/or in recovery

· Excellent interpersonal skills.

· Strong organizational, writing and interpersonal skills

· A willingness to listen and respond to others with empathy and compassion

· Experience with the mental health system in Philadelphia is preferred.

· Must have valid Drivers License

Sunday hours flexible

Monday 8am-4pm

Tuesday 8am-4pm

Wednesday 8am-4pm

Thursday 8am-4pm
