General Description:

The Foster Care Case Manager provides case management services under contract from the Pennsylvania Department of Public Welfare and the Philadelphia Department of Human Services. The Case Manager’s role is to: (1) provide case management and resource services to foster/kinship children, natural and placement families; (2) maintain case records with timely and accurate documentation meeting state/county standards and best practices and (3) provide accountable permanency placements for foster/kinship children. The Family Foster Care Case Manager has access to confidential client information and is expected to handle such information with integrity and professionalism.

Duties and Responsibilities:

¨ Responsible for providing case management and resource services to foster/kinship children and natural and placement families.

¨ Establish a working team relationship with placement families to ensure child’s well-being, promote positive growth and development and maintain stability of placement.

¨ Maintain case records with timely and accurate documentation meeting state/county standards and best practices.

¨ Responsible for providing accountable permanency placements for foster/ kinship care cases within each fiscal/contract year.

¨ Responsible for monitoring closely and facilitate through advocacy, collaboration and coaching the educational stability of children under purview, as well the physical and mental health of children and families.

¨ Responsible for identifying cases which have the potential for permanency and facilitating the outcomes by developing permanency plans, identifying barriers and teaming with partner organizations and legal resources.

¨ Prepare child/family profiles for each client/family in caseload.

¨ Participate in weekly program meetings and monthly staff meetings.

¨ Participate in case reviews, as scheduled for individual cases, and present clinical case issues.

¨ Attend trainings as required.

¨ Respond to work related emergencies after 5:00 pm and on weekends as necessary.

¨ Perform other duties as assigned by the Family Foster Care Coordinator.

¨ Maintain on-going mutually supportive, professional collaboration with the Philadelphia Department of Human Services, other state/county agencies, auxiliary services and community resources.
