Maternity Care Coalition, a dynamic $7.5M nonprofit, works to improve maternal/child health and well being.
MCC seeks a self motivated individual to fill a Case Manager position in our prison-based MOMobile program. The Case Manager will provide culturally sensitive group education and case management to pregnant and newly parenting incarcerated and formally incarcerated women. Services will be provided at the Riverside Correctional Facility and during home visits. The successful candidate will maintain contact with clients to ensure connection to needed healthcare and other community resources; provide additional support to clients needing more intensive follow-up and make referrals to appropriate agencies. Interest in maternal and child health and ability to work in a team environment desired. Bachelor’s degree in social work or related field and valid driver’s license with clean driving record required. Experience in criminal justice, drug and alcohol or child welfare systems preferred.
