AstraZeneca HealthCare Foundation Invites Applications for Cardiovascular Health Next Generation Program

The AstraZeneca HealthCare Foundation is a Delaware not-for-profit corporation and a 501(c)(3) entity separate from AstraZeneca Pharmaceuticals that is organized for charitable purposes, including the promotion of public awareness and education of healthcare issues and support for nonprofit organizations consistent with its charitable purpose.

The foundation’s signature program, Connections for Cardiovascular HealthSM (CCH), provides grants and capacity-building assistance to U.S.-based nonprofits working in innovative ways to help improve heart health at the community level.

To advance that mission, he AstraZeneca HealthCare Foundation is inviting applications for the CCH Next Generation program starting February 3 through February 21, 2020, at 5:00 p.m. ET.

CCH Next Generation will maintain CCH’s original mission to help improve cardiovascular health in the U.S. as well as many of the same criteria for funding and programs. What is new is the program’s focus on sharing nearly ten years of foundation and grantee learnings from the field so that new applicants have the opportunity to modify or adapt these learnings when designing and implementing their own innovative community-based heart health programs.

Applying organizations will need to align with one of six innovative approaches described in the foundation’s Effective Program Practices Guide as the basis of their CCH Next Generation application, or identify their own innovative approach.

Through the program, grants in the amount of $125,000 to $150,000 over one year will be awarded to eligible organizations that have never been funded by the foundation through the CCH program.

To qualify for a grant, applicants must be a U.S.-based nonprofit organization with a 501(c) designation or a public school, government entity, or municipal institution that is eligible to accept tax-deductible, charitable contributions. Organizations that are 501(c)(3) and also have an IRS designation as a 509(a)(3) are ineligible for funding.

CCH Next Generation welcomes applications from every state in the nation and has a preference for funding applications from the top 20 states by cardiovascular disease mortality (Centers for Disease Control and Prevention, 2017 data). Further preference will be given to applications from those states where the foundation has never previously funded a program.

Organizations who receive funding in the 2020 application cycle may be eligible for a second year of continued funding in 2021, based on performance and assessment by the foundation’s university evaluation partners, program staff, and board of trustees.

The foundation will host an informational webinar on January 16 at 3:00 p.m. ET to help interested applicants prepare for the open call for applications.
